About Changing Your Name on Your Louisiana Registration

Before you may change name on car registration in Louisiana, you must notify the Social Security Administration (SSA) of your new name and apply for a replacement Social Security card. Then, you may change car registration and driver’s license information with the Louisiana Office of Motor Vehicles (OMV). Changing your name with the SSA first is important because since the OMV will verify your new name against SSA records.

To notify the SSA of your new legal name, you must complete a Social Security card application. Submit the completed application to your local Social Security office along with proof of your name change, identity and U.S. citizenship status. After making this change, you will receive a replacement Social Security card that displays your new legal name.

Your Louisiana Name Change Checklist

To perform a name change on car registration materials after updating your information with the SSA, you must first correct the information on your Louisiana driver’s license or identification card.

To update this information, visit your local OMV office to submit certified proof of your name change, such as:

• A marriage certificate.
• Court order.
• Divorce decree.
• Adoption certificate.

If you need to purchase a certified copy of these legal records before submitting your request, you may do so through the Louisiana Department of Health (LDH).

When making your DMV name change, you must also present your current Louisiana driver’s license or ID card, proof of automobile insurance and evidence of state residency. Then, you must pass a vision screening and provide payment for the duplicate driver’s license.

After updating the information on your driver’s license or ID card, you must change the name on your other OMV records, such as your vehicle registration and title certificate.

Typically, you can change name on car registration and titling credentials immediately after updating the information on your driver’s license. Then, you may apply for a replacement registration certificate that displays your new legal name.
